(1) This show that ground commanders that are directly in charge and involved in the safety of their men and soldiers - will be responsible - if safety lapses occur that lead to injuries or death of their men and soldiers.
(2) Individual soldiers will also have to observe safety procedures to prevent accidents from occuring.

U mean the safety officer & OIC of the exercise were not involved at all and are deem innocent?

If you had followed this incident on the news more closely, it was mentioned that Aloysius himself presumed everything would be fine & have enough time to get out of the way of the barrel during its lowering. Therefore, it was a large act of negligence on his part too. Of course it could have been avoided if these 2 servicemen had known the procedures to stop the barrel from lowering any further, but don't just jump the gun & blame it all on the two of them.
