Every level - from top, middle to lowest level - should play their part.

If anything go wrong, we should examine which level go wrong and which officers did not carry out the due diligence in the conduct of their work.

Accountability will be attached to the level and officers - who fall short.

This is to ensure "complacency, incompetency or slack" - will not occur or recur.

Another thing that is correctly brought out :-
Mr Heng said the country is where it is today because its pioneers took risks. “If public officers had not dared to take risks for fear of being axed if things went wrong, we would never have built an exceptional country.”

As a progressive Nation, we must dare to :-
(1) experiment new things
(2) do pilot testings of new tech, new ideas, new concept - in a sandbox before rolling out in phases and to the whole Nation --- so that the Nation can progress leap and bound

--- This mean we will have to take calculated risk --- a risk by studying every aspects (to minimise or eradicate danger, failure etc) --- humanely possible.
But in the event we fall short (humanely unexpected) ---- we will have to learn the lesson and put things right.

This is where I agree on the statement :-
"But we should not “routinely dismiss officials” whenever things go wrong, as doing so may give the appearance of solving the problem when that is not necessarily the case, he wrote".

---- Else no civil servants or politicians will want to or dare to "try new concepts, ideas, new tech" - to bring improvement to Singapore and the lives of Singaporeans --- because "they will get fired if anything go wrong".

When this happen - Singapore will not progress so rapidly --- we will still be living in "our kampong world".

Officers should be taken to task if he/she :-
(1) is negligent or careless or "heck care" - without doing the due diligence
(2) by failing to comply with established procedures, guidelines or processes - that are designed to prevent failure, or danger
(3) failing to carry out various levels of check, verification - by various level of officers - meant to arrest failure, or danger.
(4) plain incompetency - unable to do the job or duty.

But having say so ---- we should also provide chances and opportunities for anyone to repent, recover and recoup themselves from failure - so that they can improve or make amend from past wrong ---- and not condemn them forever.

Given the fact that those people at the highest position cannot prevent mistakes done by their subordinates but isn't it that as their chief and someone who is responsible for the smooth running of the entire organization of which they helm must also be accountable for the problems arise from mistakes done by their subordinates? So Mr. Heng, is there actions to call for any accountabilities from any of the top ranking ministers for all those mistakes so far? If our govt do not have a strong and disciplined culture of accountabilty and shared responsibilty values in dealing with mistakes that affected the reputation and creditabiltiy of the nation and more importantly the citizens then I guess, problems like these would continue to be as per normal and one of those things that could happen that no body wants it to happen!

Each ministry may have 10,000 people working in it. 10,000 people every day sure at least 1 person will make 1 mistake. So the minister must be accountable for every mistake made each worker? Like this, every day we must change one minister issit?

Richard Kee, I am refering to those mistakes like those occurred recently that were shocking, causing embarrassment to the nation and it should not have had happened like causing deaths or huge financial losses to the nation and all citizens at large.
