Health Minister Ong Ye Kung shared that Singapore could be hit by another wave of Covid-19 in July or August, and every healthcare setting must be prepared to handle such patients. Singapore is preparing for a Covid-19 wave driven by the BA.4 and BA.5 Omicron sub-variants, which were first detected in South Africa.

💬 Are you concerned about the possible new wave of Covid-19? How confident are you that Singapore is prepared to combat the new wave as we continue to re-open?

Minister Ong noted that cases in South Africa peaked at a lower level than the original Omicron wave, and there was no sharp rise in hospitalisation and death rates. Moreover, as vaccination coverage is high and mask-on rules are still in place, he believed that Singapore can afford to be "quietly optimistic”.

He said the country also needs to make more hospital beds available, in case pressure builds up again. Hospitals have rolled out home care services, freeing up beds while allowing people to receive care in their own homes. Ramping up the number of nursing home beds to take in long-term hospital patients who are waiting for places in nursing homes will free up hospital beds as well.

Minister Ong reiterated the importance of convincing eligible seniors aged 60 and above to get their booster shots as they will be the most vulnerable. Around 12% of people in this age group have not yet done so.

Now? 

Just when the world is getting used to the Ukraine war and accepting the unavoidable inflation of goods and foods. Now comes BA4 +BA5 +, Monkeypox! 

What's next? 

Small & Medium Enterprises will continue to suffer and surviving should there be a new variant of scare that leads to intermittent tightening of measures or quik lock down say up to 14 days or longer? 

What's going to happen will be the shorten lifespan (to the verge of no runway) of these small (xxxs) to smallest (s) enterprises that add up to the SME, that S part will 'die' matter of timing? 

Will it be like many little dominoes collapsed and causing the larger pieces to fall too? 

Tomorrow? 

Could all the 'hansaplast' of stimulus save these SMALL ones from 'bleeding' ? Should this domino effects happen there will be structural unemployment, Stagflation and stagnation for many nations? Remember, it is a highly connected world with it economics or pandemics like a domino set !

Many of my friends / family have caught it in the last few months - here and abroad - and none have had more than mild symptoms. I think it is something we will have to live with and probably have annual vaccinations for. I respect that others have different views about the severity of the disease and hence the need for masks etc.

3. Notice that, there are still 8% of the population that are not vaccinated.

4. Get the remaining 26% vaccinated group - to go for the 3rd shot booster to get maximum protection.


5. High vaccination is our best bet to combat covid - regardless of the variants.


6. Lastly, MASKs is our best "personal distancing tool" to give us the "last line of defense" against covid - despite full vaccination - as Mask block covid and its varaints from infecting our nasal, mouth and lungs --- the primary means of infection.

Barring the vaccines cannot block covid infection - but provide protection against serious illness, hospitalisation or death ---- MASKs is our best safeguard to block covid infection - to protect ourselves and the people around us.


7. Hence, if Omicron sub-lineage 4 and 5 surge in July and August - one sensible measure is to re-impose mask for indoors and outdoors --- without re-imposing other restriction measures - so that our Economy, business and jobs can keep going.


8. In the longer run, medical community should put in more resources to develop nasal spray that can effectively block covid infection regardless of its variants.

"Scaling and Preparing"

Fixed - the number of sick beds, medicines and basic equipments can be scaled upwards. 

Variables - the knowledge of medical staffs from nurses to doctors to specialists are not easy to scale up given a fixed time line. How about other chronic diseases popping up? 

Example of reallocating resources to nursing homes to handle covid cases among seniors and newly admitted cases will be a variable for these premises and their staffs? Are these staffs well trained for the treatments of mild covid and into the next stage of covid illness? Should a case escalate into serious stage, will this patient needs to uplift to regional hospital? Where are the necessary equipments if this patient remains at the nursing home ?

How about deriving an infection model to say up to 1% of 5 million population will be infected and need hospitalisation of a minimum 6 nites stay? From this 5,000 covid patients will have 150 that need ICU interventions comes with specialised cares, targeted equipments and medications. 

Summary:

Die at home or hospital or elsewhere, it can be a patient's choice or last wish. It can also be that stage in life whereby painless care is crucial for that patient to depart this world in peace than continuous sufferings in pain.

Lastly, many homes these days are not equip and can be costly to care for their loved one's last journey. Mainly, the daily lifestyles of many of us is a 'busy and buzzy' one! How to adequately care for one dying person, it is a sad, emotionally down and a sense of inseparable moment for this family!

Simply put it, let's leave it to the patient's and or the family to decide.

Fixed - the number of sick beds, medicines and basic equipments can be scaled upwards. 

Variables - the knowledge and skill sets of medical staffs from nurses to doctors to specialists are not easy to scale up given a fixed time line. How about other chronic diseases popping up? 

Example of reallocating resources to nursing homes to handle covid cases among seniors and newly admitted cases will be a variable for these premises and their staffs? Are these staffs well trained for the treatments of mild covid and into the next stage of covid illness? Should a case escalates into serious stage, will this patient needs to uplift to regional hospital? Where are the necessary equipments if this patient remains at the nursing home? 

How about deriving an infections model to say up to 1% of 5 million population will be infected and need hospitalisation of a minimum 6 nites stay per patient? From this 5,000 covid patients will have 150 that need ICU interventions and staying period of up to 30 days or longer? The ICU comes with specialised cares, targeted equipments and medications. 

Summary:

Assess snd access the possibilities, the human resources, knowledge based, medical equipments and lastly dry runs and derive a sound covid medical care model co-run with the existing medical needs.

Cares and Caring? 

Die at home or hospital/hospice or elsewhere, it can be a patient's choice or last wish. It can also be that stage in life whereby painless care is crucial for that patient to depart this world in peace than continuous sufferings in pain.

Lastly, many homes these days are not equip, family members have not experience this end of life skills set and can be costly to care for their loved one's last journey. The daily lifestyles of many of us is a 'busy and buzzy' one! How to adequately care for one dying person, it is a sad, highly emotional and a sense of inseparable moment for this family!

Simply put it, let's leave it to the patient's (of all ages) and or the family to decide.
