New rules forbidding unvaccinated people from dining in at hawker centres and coffee shops kicked in on Wednesday (Oct 13), with similar restrictions to take effect in malls and large standalone stores in a week (Oct 20). 

NEA, which manages hawker centres, and the Singapore Food Agency, which handles coffee shop operators, had earlier said that they will be working with stallholders to advise unvaccinated patrons against dining in. Checks on the vaccination status of diners will be done by safe distancing enforcement officers, who will do this selectively as part of their patrols, said the agencies.

๐Ÿ’ฌ How do you think we can protect the vulnerable and unvaccinated while ensuring minimal disruptions to daily life? How often do you do and your family do your Antigen Rapid Tests? How are you adapting to these costs of protection?

MTI and Enterprise Singapore (ESG) had earlier clarified that exceptions will be made for those who can show proof that they need access to childcare or medical services within malls. However, these concessions will not be extended to unvaccinated caregivers of children attending tuition or enrichment classes in malls, unlike those going to childcare centres and pre-schools.

A spokesman for Enterprise Singapore (ESG) said on Thursday (Oct 14): "Childcare centres or pre-schools are key services typically required by parents who need to return to work."

Since the new rules were announced, clinics and private healthcare providers have also reported an increase in people coming forward to get their jabs and booster shots.

That's why I strongly support the government vaccination differentiated measures to nudge the last harden group to vaccinate.

We need to be tough to make them vaccinate to save themselves and our society and not subject disproportionate burden to our healthcare system, finance, and Economy.

Agree.

I know there are some grey areas that some may genuinely not suitabe - but thave are a very small minority and can look at case by casee basis. Everyone is treatable - as there are many options out there - just have to pick the right one.

But we need to be tough on those frivolous majority that are eligible but refused.

We should plan to address this frivolous majority first and then look at the gray area.

Now most anti-vaxx are holding onto the gray area real concern to press their own frivolous case.

Thus there is a need to get tough on this majority eligible but defiantly refuse to vaccinate - and held everyone back.

Agree.

My position basically cover the eligible but anti-vaxx and the eligible elderly.

Pregnant one I did not press the case - though they are encourage if doctors say they are eligible.

But pregnant women are vulnerable - as they carry the lives of their baby and themselves.

As they are not vaccinated, they need to be kept out of circulation as the daily death spike - to protect themselves and their babies.

The same go for elderly and adults not vaccinated - the vaccination differentiated measures - is to keep them out of circulation - so that they won't get infected and succumb without vaccination protection - as daily death and hospitalisation spike.

Yes.

That's the challenge the government is facing.

So I think the government is taking the current vaccination differentiated measures seriously.

I think this is our last chance to resolve the vaccination of the last harden group. 

The louder the noise they make - the greater the indication the measures are working. Important for the Government not to cave in - and press on. I think we will see good vaccination results among this group more than 1 month later.

Specialist won't want to mandate vaccination for the seriously ill elderly.

They will just advise the patients that vaccination will protect them.

It is still up to the patients and their families members to make the decision.

Thus vaccination differentiated measures - want to force them into a corner and make their right decision for themselves.

Without forcing them to make their choice under a do or die situation - they will just procastinate.

Thus I feel that some will give in and vaccinate.

While some will decide otherwise - and retreat into their hermit corner.

But either way they are protected from the deadly spike pandemic.

Anyway, seriously ill elderly should be kept out in circulation in the mall and dine-out - as if they are infected - they will very likely to succumb as casualties.

Moreover, if they are seriously ill elderly, they are not likely to be so healthy to walk around the mall and dine-out.

So vaccination differentiated measures are appropriate for them - as we are seeing few thousands spike per day, hundreds hospitalised per day, hundreds in oxygen support/ICUs and big sinlge digit and double digits death per day.

Yes agree - for some of this very serious cases.

But logical deduction short of official statistics that all these gray areas are only in the thousands I believe - not the 77,000 defiant that are eligible but refused for whatever frivilous reasons.

It is this harden but frivilous majority latching onto these few thousand minority's genuine case - to press their frivolous cause and refuse to vaccinate - but put a tremendous burden on our Society, our healthcare system and our National finance.

It it this frivolous majority we are going after and not the minority genuine cases. 

Minority cases still can be dealt with after we handle the majority harden group.

There are so many vaccine options out there and there must be one that is bound to be suitable to the small minority genuine cases.

If not mRNA, we have the non mRNA - inactivated virus, protein sub-unit and others. we also have the nasal spray, the merck oral prevention pill, the monoclonal anti-body treatment drugs and many others.

If this minority are so fragile that not even one is suitable (which I seriously don't believe) - they won't have survive by now - because they will not also be able to take other medications for their current medical conditions.

That's why I say the last harden group not easy to deal with and very painful to resolve. The key is "Don't cave in. Press on".

Thus we need combination of selflessness, wisdom and compassion to resolve it.

1. MOH Ong YK indicate that Singapore is on trajectory to achieve 90% vaccination. 90% is a good target. But is it good enough?


a. 90% is good, but not good enough - because it means the last 10% adults especially elderly are not covered. And especially elderly are the most vulnerable and need full vaccination to protect them.

Some people say 90% is good enough no need 100% - does it means the remaining 10% are expendable and can be sacrificed - especially if they are your loved one?

Of course not.

Hence 100% vaccination must be the goal to cover everyone - as even 1 death is one too many.


b. Small pox R0 = 5 to 8 and chicken pox = 8 to 9 ---- all required 100% vaccination to curb the pandemic. This is past empirical evidence.

US study reveal that the R0 for Delta = 9, Malaysia condition exceed 10 (where one infected person can transmit to 9 or more person).

Correspond to smallpox and chickenpox pandemic ---- this means that to curb Delta covid where R0 = 9 or exceed 10  --- then only 100% vaccination can curb the covid pandemic - as per past pandemic.

What is so difficult to understand - to correspond Delta covid outcome to smallpox and chicken pox outcome - that we need 100% vaccination to curb the covid pandemic?


c. Another justification to support 100% vaccination is that if one acquire Superhuman Immunity, infection number drop significantly, so are hospitalisation number, oxygen support, ICU and death.

This is illustrated in real scenario where 3 Countries demonstrate such traits that have acquire Superhuman Immunity :-

i. Israel acquired Superhuman Immunity through :-

SuperHuman Immunity = 2 jabs vaccinated immunity (Pfizer-BioNTech) + 3rd jab booster (Pfizer-BioNTech).

- mark by significant drop in infection number, hospitalisation number and death.


ii. Demark and Norway acquired Superhuman Immunity through :-

SuperHuman Immunity = Natural Infection Immunity + vaccinated immunity 

--- but note :- both Denmark and Norway suffer high casualties - death and hospitalisation with multiple spikes - before they stabilised after a big majority of the total population acquire SuperHumanity from infection follow by vaccination.


--- Thus 2 jab vaccination is the underlying foundation to acquire Superhuman Immunity.

- So does it means that the remaining 10% don't need 2 jabs vaccination and can acquire Superhuman Immunity (when vaccination is a pre-requisite to acquire Superhuman Immunity)?

- It is a no-brainer that 100% vaccination is a pre-requisite to acquire SuperHuman Immunity - if Singapore want to transit to endemic safely.


c. Some say allow Medical conditions to be exempted from Vaccinated Differentiated measures.

- Does it means that without vaccination - this medical conditions people can survive a covid infection when they are allowed to roam freely in mall, remove mask to eat in food courts and hawker centres - and won't succumb to covid?

- The disproportionate elderly death reveals that unvaccinated elderly are the one that die from covid infection and many come with medical conditions.

Isn't it all the more this group of people should be protected and not allow to roam mall and dine-out freely?

- There are so many types of vaccines out there - be it the mRNA types, non-mRNA types like inactivated virus, protein sub-unit and many others.

There are bound to have one type that is suitable for each and everyone. Else I can't see how this medical conditions people can take other medication to survive - as any one medicine will come with some side effects.


2. Thus I will like to reiterate a point - whatever lame excuses the anti-vaxx people want to come up with to want to get exemption from vaccinated-differentiated measures --- Government should stand firm and persevere. The anti-vaxx people only made up of 5% of the total population - they are of a very little minority - but very vocal.

Stand firm, resolute and don't cave in.

The louder the voice we hear from the anti-vaxx --- -the higher the chance we will get the anti-vaxx to vaccinate - because the vaccinated differentiated measures has obviously pain them.

We are closer to 100% vaccination - as long as we don't give in - and all the more tighten the screw further.

No pain no gain. 

Until we see 100% vaccination - then we can celebrate as a Nation!

How to derive the 100% vaccination?


((R0-1)/R0)/% vaccine efficacy

= ((9-1)/9) /  88%

= 101%


Note :- vaccines efficacy according to various study - local and overseas range from 40%, 50%, 66%, 88%, 95% - depending on when the vaccination is taken.


I took 88% the higher range for calculation - and it say >100% vaccination for total population is required!

We are lucky that we have made amazing progress in Singapore. Europe and Nth America are large land masses, wealthy and have safety in numbers. Singapore is tiny, so we are able to manage, but we are surrounded by Countries who are large, and developing so not able to cope. We have a unique position and need a unique solution. It would be helpful and beneficial for MOH to share the nature of all co-morbidities as just like vaccination, knowledge is power. In this case the power to control our destinies and lead healthy lives by making smart choices. 

Unfortunately Singapore has one of the highest rates of chronic illness / comorbidities in Asia. Namely high BMI, Diabetes, hypertension and cancers in their various forms. When you succumb to illness it’s often a reflection of biological not chronological age (ie an unhealthy 20 year old may have the body of a 50 year old).…

Although those comments would've been made with an element of scorn and belittling, it would definitely be useful to know what conditions these young patients suffered from which contributed to their untimely demise.

Alcohol overuse (more than 2 standard drinks per day for men and 1 for ladies.. messes up mine response) 

Pre-diabetes (means sugars not high yet but body is starting to feel the strain .. pre-dates full diabetes by 10 years) 

Hypertension 

High cholesterol (an indication of chronic ill health even without symotons)!

As the stallholder was giving me my drink, the boss lady asked abt my Vax status if I was dining in.

I showed her .

So,  only the drinks stall lady felt compelled to do so but the rest of the stalls…

Comparisons are not meaningful without context. Singapore is a very small and urban city state without a real hinterland, and should never be really compared with the likes of HongKong. It cannot even be compared with Israel as Israel’s territory is larger. Singapore is unique in its situation and vulnerability so our solutions have to also be unique to us.

Personally, I agree 100% vaccination is the target goal (I.e I don't think govt goes out to say "let's aim for 85%"), but realistically, the number will be lower after accounting for those ineligible and those unwilling. 

The question is whether we should make it mandatory, and I personally feel we need not so so at this juncture. Some people do have legitimate concerns, some are just antivax. Regardless of the reason, mandating an experimental vaccine might be overly draconian at this juncture. Yes, the pros might outweigh the cons, but the cons might mean more to some others. We can encourage, we can disincentivise, but ultimately It should be…

How Covid clean is a nation? 

For the purpose of discussion, a fiction, a country will not retain its foreign visitors from leaving due to positive test results at the last 24 hours or critically 4 hours at the airport prior to checking in and boarding the airplane, the cruise ship or the tour bus to return to their home country.

Why this country will ensure that all departing visitors "passed" the swab tests? 

Simply in two words "REPUTATION & HOSPITALISATION"! Would a country strictly identify positive cases out of these thousands visitors that will subject to quarrelling and the ultimate issuance of the Quarantine orders? What's the visitors overall experience of this country? What's the frequency of revisiting this country? Were the Covid measures overly secured or laggingly loose? Are hotel frontliners showing warm, confidence and able to communicate and connect in their daily interactions with the guests? 

What's risk and is it risky? 

Assuming a daily arrival of 10,000 visitors, what is the RISK of infection from these valuable visitors? We assume a 1% hit rate that is 100 visitors for every 10,000 arrivals over a staying period of up to 7 days or more. We further stratified infection risk from 1% risk to 0.2% which is 20 visitors per 10,000 will need hospital needs couple it with a very low percentage of 0.5% cross border contaminations into the host country's domestic population, this case C-19 variant. Is this model risky? Perhaps no, simply under covid situation the actuarial science has the metrics to verify and support such risk, this case the first step in the right direction of opening up! 

Safety Tools:

Visitors - - 

The Tourism board should initiate a briefing and education program for our tourism frontliners at airport, attractions and accommodations level. To train many smart clusters of tourism frontliners to confidently project our core abilities in handling Covid or crisis in a steady logical manner. 

How to attend to visitors' enquiries, challenges and needs related to C-19? Make available on Internet and relevant places, a simple little folded paper brochure showing SHORT steps to mitigate C-19, crucial hotlines including 995/999, and all embassies phone numbers, emails and address. 

Home Ground - - 

A travel and handy Covid First-Aid kit measuring 32cm x 32cm x 8cm made of plastic material either compressable or a plastic box. Budget up to 1 million kits for families and offices. 

In it are compartments to fit a Thermometer,  an Oximeter,  a few ART,  one dozen Masks, Small refillable bottle Hand Sanitiser, Wet wipes and Paracetamol tablets, importantly a simplified SOP in mitigating Covid risk and instruction sheet for further actions and crucial hotlines. 

Living with Covid is to accept the convergence of risk and risk appetite that is to let things function and loosen the grib in managing risk towards an optimise level, to be specific this case are economic activities, medical capacity and public health. A few bold step at a time, perhaps a ten minutes quik haircut in the mall is safe and eventually full services /amenities to all that's smart and inclusive!

To sum things up,  converge and unify safety tools and simple instructions for our visitors and our citizens. 

A country REPUTATION of being clean and covid safe will be the keys in the success of its implementation of cross border travel initiatives that will consequentially be a muscular driver of economic activities, trade progressions & integrations and differentiates from other nations. Being unique, be different and avoiding a habit of "monkey see, monkey do" approach: importantly to seek every small advantage, that's simply an "unfair advantage" over others! 

A brave new world, afterall, 99% of all visitors and locals are potentially asymptomatics? Let's open our minds, lightened our hearts and embrace a reset - a new norm of FUN, FREE & EASY experience for ourselves and our visitors!  As the old saying goes " the rest is history...! "
