This is more serious than train fault.
Should have sonar, sensor or rf in place for moving train to detect another train or stationery train - and trigger emergency brake to stop the moving train. This will prevent collision.
Also if possible they could also detect moving objects or foreign objects like human on the track at a distance to trigger emergency brake.
