πŸ’¬ What are your views on the latest announcements, particularly the plan to accelerate the vaccination programme to progress the opening up of the community? What can we do to encourage those who are eligible but who have not been vaccinated to do so?

Yesterday, the Multi-Ministry Taskforce on COVID-19 announced that Singapore would accelerate its national vaccination programme from Saturday (June 26). 

The MTF urged people over 60 to get their jabs as soon as possible, noting that “not enough” seniors had been vaccinated. Minister Lawrence Wong also called on younger Singaporeans to encourage their elderly family members to protect themselves through vaccination. 

MOH noted that vaccination remains a key enabler, and its ability to help Singapore re-open safely can only be felt when we can collectively achieve a high level of population coverage for vaccination.

Phase 1 – Voluntary 

Get the media, social media influencers, experts etc to spread propaganda. 

Phase 2 – Incentives

Offer lottery tickets, free beer, free donuts, etc. to those who are vaccinated.

Phase 3 – Denial of Access to Venues and Services (Private Sector Punishments)

Use corporations to deny people access to services (such as air travel, cruise ships, restaurants, sporting events, concerts, etc.). This phase hopes to make being unvaccinated extremely inconvenient.

Phase 4 – Criminal Offence (Government Sector) 

This phase will kick in after the media blames the unvaccinated for continued outbreaks that are killing people. Any who refuses to vaccinate will be fined or jailed. The G will need to suppress the news about those outbreaks that occur in clusters that are mostly vaccinated. For future cases like the TTSH and MINDS clusters the G should lie that most staff and patients there were unvaccinated because they refuse vaccination. This is lying for the greater good.

Phase 5 – Medical Rape

This kicks in as the unvaccinated would rather pay fines and go to  jail. Empower the police or even the army to break into people’s homes and drag away those who are not vaccinated. Strap them to chairs or gurneys and force the needles into these people.

1. Difficulty in getting elderly vaccinate.


2. These elderly are not the anti-vaxx - as they don't stop their families members from vaccinating. They don't spread false information or discourage others from vaccination or spread fear claiming vaccines will harm people - like those anti-vaxx group do with malicious intent.


3. But they refuse to vaccinate out of fear :-

a. Either they have read online false information spread by the anti-vaxx group about the safety of the vaccines.

b. Or they read about coincidental death after vaccination - although the death may not related to the vaccine.

c. They have some discomfort due to some old man sickness - and fearful the vaccines could make it worst.


4. Government send people down to persuade the elderly to vaccinate to protect themselves and their families members.

But they refused giving the following reasons :-

a. They are not feeling best of health and still are on some long term medication - they are fearful the vaccination will make it more discomfort for them.

b. They say, are you going to be responsible if anything happen to me.


5. Children of elderly take turn to persuade, coax and encourage the elderly to vaccinate themselves to protect them from covid so that they won't fall into serious illness --- but the elderly excuse is - as long as I stay at home and don't go out - I will not get covid.

- But the truth is that, children and grandchildren who work or study could be infected (even after full vaccination) - and may infect the elderly who are not vaccinated.

- But elderly still adamant not to get vaccinated - out of fear.


6. Thus I suggest the Government to impose "Mandatory Vaccination".

a. If need be medical staff escorted by 2 policeman visit elderly at the home to get "Mandatory Vaccination" - armed with warrant that say "Vaccination" is "Compulsory" - and must vaccinate on the spot after examination by medical staff to be fit and eligible for vaccination.

b. Then put a jab into the arm ----- authorised by "Law".

Children who are not vaccinated don't get equal access as other children of their age Eg school admissions which require proof of vaccination. This is the manner of enforcement, NOT force poke!! The law is not here to violate bodily autonomy like that. Bodily autonomy is a cornerstone of medical practice. So even if you do manage to pass a law like that which is not found unconstitutional, no medical professional will be prepared to do it.

Singapore will accelerate its national vaccination programme from Saturday (June 26) and administer up to 80,000 vaccine doses daily, said Health Minister Ong Ye Kung today at a virtual press conference held by the multi-ministry task force on Covid-19.

The ramp is due to the delivery of vaccine supplies being brought forward, and the Ministry of Health (MOH) expects to add another 500,000 new slots for people to get the first dose of the vaccine in the next few days from now till mid-July.

To allow more time for Singapore citizens aged 12 to 39 years old to make their vaccination appointments, the two-week priority window for this group will be extended by one week, till July 1.

Singapore has also signed advanced purchase agreements with American biotechnology company Novavax to secure its protein-based Covid-19 vaccine, with supplies possibly arriving before the end of the year.

The MTF also said with higher vaccination numbers, Singapore will be looking to live with Covid-19 as an endemic disease. Mr Wong said the task force is working on the “potentially revised public health guidelines” that would apply to vaccinated persons and will announce them when ready.

3) Up to 4 Cisco and 4 ICA officers standby to receive and direct arrivals to the designated buses to the respective HOTEL for 14 days Quarantine.

4) ICA officers to count & collect Passports only when passengers board the buses. Other immigration papers to be dealt at Hotel ICA counter. 

5) Luggages to be fumigated at conveyor belt out from the aircraft before been stored into the buses luggage bay.

6) MOH swab team standby at Hotel to perform ART/PCR tests. Positive Cases to be transfered out. Tentage, tables, chairs and fans to be erected at site. 

Reason:

"In the positive spirit of safety for all..." 

To minimise physical contacts with the main airport staffs and other passengers. 

The aircon Airport main halls will be utilised for LESS RISKY countries and / or with proof of true vaccinate passport of passengers! The copies of the arrival passengers manifest will have information on these details. 

This method of diverting and minimising risk are no means of discrimination nor disrespectful to the HIGH RISK countries of arrivals.

2. A roadmap on how the Endemic stage is to be achieved - need to be mapped out - in particular what are the milestones involved.

Eg. 50% full vaccination by when?

75% full vaccination by when?

80% full vaccination by when?

100% full vaccination by when - eg. intend to impose Mandatory Vaccination to achieve this target?


3. Assume the vaccination target to reach the inflection point is 80% - what is the Governent approach in getting the rest of 20% vaccinated?

- Bearing in mind, 20% population represent 1,130,000 people unvaccinated. Potentially a large group that will cripple our hospitals and ICUs.

- I support Mandatory Vaccination if Government has no difficulty or politically able to do so without triggering backlash.


4. What treatment methods are in place - to treat those infected with covid that display symptoms in hospitals?

- That will make oxygen support, hospitalisation and death - unlikely and result in very successful recovery rate?


If the roadmap to Endemic stage can cover all the above - it will give Singapore population very good confident that Singapore is ready for Endemic stage --- and the Singapore population are well protected from the covid irregardless of variants.

I've been following most of the full press conferences and it particularly the last 3 press conferences have seen the media being restricted/limited to just 1 question per media. In fact during last week and today's press conference, the emcee even (rudely; if I may add) interrupted the media asking the second question. I find this to be rather unbecoming, disrespectful and impeding the media from playing their role. As public citizens, we are unable to directly ask questions to the MTF and so the media plays the part of asking on our behalf - why should it be that they get cut-off for asking more than 1 question? Where is the accountability to the public?

The cautious approach to reopening is to allow the authorities to "buy time" so more people can be vaccinated, said the multi-ministry task force tackling Covid-19 on Friday (Jun 18). 

"We are taking a more calibrated, more careful approach - not swinging to either extreme of shutting down, restricting very tightly all the different activities, nor opening up too recklessly, but taking a more careful approach of allowing some resumption of activities, still with appropriate safeguards and precautions in place, and buy us time for our vaccination rates to go up," Minister Lawrence Wong added.

Asked about the vaccination targets, Mr Wong said that there are two key milestones: 50 per cent of the population to be fully vaccinated, which he believes can be done by August or so, and 75 per cent of the population to be fully vaccinated at a later date. The task force will progressively ease restrictions both within Singapore and at its borders based on these two milestones, he added.

My close friend comments:-

We should discern between being reckless and taking a risk-based approach. 

As our vaccination rates rise, we should look at the new metrics for reopening rather than mere numbers of new infections.

Higher infection  transmission, more serious and fatal cases are worrying trends. But high percentage of age 45  and above being vaccinated; better contact tracing capabilities; high testing capacities; and availability of affordable good surgical masks can help mitigate those risks?

We are learning to accept that zero-covid is unachievable, at least not in the near term. Then can our goal be near-zero deaths and serious illness? Then how much more can we stealthily reopen without the virus rearing it's ugly head?

Conditions for opening of our Economy (and declare endemic stage) - as more contagious Delta variants and possible other variants hit us.


1. Live and Livelihood - both must be ensure and balanced.

- This is the uncompromise position we must take in opening up our Economy as more contagious strain hit us.

- Reckless opening will take more human lives.

- To restrictive measures to protect lives - will hurt our livelihood.


2. Thus Calibrated reopening of our Economy - in tandem with infection rate, hospitalisation number, Oxygen support number, ICU number and death number - should be model upon.

- Eg. weight can be given to :-

a. infection rate

b. hospitalisation number

c. Oxygen support number

d. ICU number

e. death number.


3. Vaccination number must also be part of the equation.

- Noting that UK has a 59% 2 dose vaccination and 81% 1 dose vaccination - and yet cannot stop the Delta variants spike, hospitalisation and death.

- By using transmission rate, UK study projected that only beyond 80% 2 dose vaccination - will only stop the Delta variants --- and get themselves ready for "Endemic stage" - where covid is treated as "mild flu". (Source :- UK study of transmission rate based on Delta variants)

- Singapore 2 dose is only at 35% to 36%. Hence we cannot declare "Endemic stage" like some suggest as we are still far from the projected "inflection point" of 80% based on UK study.

(In fact, Singapore projected 2 phases - by end Aug 50% and the next milestone 75%) - Hence calibrated gradual opening will need to be put in place - taking into account point 2 and point 3 - vaccination rate.


4. Treatment medication must be ready --- if infection number is to be diminshed in significance while emphasizing zero or minimal death, low ICU, low Oxygen support and lowe hopitalisation number.

a. Current ongoing trial of oral polmerase inhibitor pills on trial by a couple of US based pharmaceutical companies such as Pfizer etc (Source :- news article - as attached).--- have proven good potential as it drastically reduce covid virus load within 2 weeks - and turn severe cases into mild cases.


b. Cocktail anti-bodies drugs currently used in India - has also good potential to treat severe cases and turn it into mild cases. (Source :- news article - as attached).


Only when promising treatment drugs are available and have potential to treat severe covid cases - and prevent death, reduce ICU, oxygen support, hospitalisation cases --- can we safely enter into "Endemic stage" - and treat covid-19 as mild flu - regardless of the variant strains.


(Only when point 1 to point 4 is met --- we can safely declare entering into "Endemic Stage".

While progressing along the way to endemic ---- we should use graduated calibrated approach to reopening our Economy - guided by the above parameters. A calculated cautious opening and not reckless opening).


5. Understand that the Government has difficulty in mandating "Compulsory Vaccination" --- as some reports surface that parents feel pressure to sign up their children for vaccination - and MOE have to issue statement to say that vaccination is voluntary but encouraged.

- I feel that Government has adopted the right approach - to do :=

a. Provide information, encouragement through media by Ministers, medical experts, celebrities, peer pressure etc.

b. Adopt "soft nudge approach" first - to achieve the milestones of 50%. then 75% full vaccination - and then it will be harder and harder ---- as we face last group of "strong headed population" who are either anti-vaxx or misguided individuals, or fearful individuals or some who have medical problem.

c. Then Government will have to readjust its strategy - to adopt psychological approach - can be soft, hard, inconveniencing non-conformers and if all else fail consider "Mandatory Vaccination" as the last resort. 

This is using the "layer by layer approach" - to achieve vaccination outcome - like peeling an onion until we reach its final core.

(Currently, Government can make swab test "Mandatory" - when a cluster is identified.

Wonder, will Government able to convince the last harder group - to vaccinate through "Mandatory Vaccination" ---- if swab test can be make "Mandatory" without backlash?

I feel that if use it at the right place, right time, right conditions and right settings ---- Government may be able to pull through with "Mandatory Vaccination" ---- when approaching the last group.

Because even if 10% out of the 90% unvaccinated ---- we are facing a whopping 565,000 unvaccinated people --- a "Big Trojan" that can cripple our hospitals and ICUs.

As of 12pm, MOH has preliminarily confirmed 13 new cases of locally transmitted COVID-19 infection. 

Based on investigations so far, the cases are in the community, and there are no new cases in the dormitories.

There are 3 imported cases today, who have already been placed on Stay-Home Notice upon arrival in Singapore. In total, there are 16 new cases of COVID-19 infection in Singapore today.

MOH will share further updates in its press release that will be issued later tonight.

Further pointers to my previous suggestion the idea of Rings of Fire, Epicenter and Zones in identifying individuals and importantly potential clusters.

ADD ON:

From a SINGLE block data to the vicinity, putting aside amenities and facilities, we will be one step closing in on the infectious cases, in transiting cases and potential clusters faster than previous methods of contacts tracing! 

Perhaps, using the latest vaccination data, we will be able to ZOOM into detailed numbers of vaccinated people with say a block of flat. We then expand this block into other  blocks within 1km, 2km, 3 km and so on in deriving a percentage of vaccinations at each range of diameter! Targeted CB, Swab Ops and the necessary supports will be well and hopefully ahead of time (3-5 days advantage). Importantly, a 

minimum level of 50% confidence in grabbing tightly the situation, this case to contain covid! 

These detailed numbers and data are STRICTLY confidential and NON BIAS! 

The idea is HOME, we homed in and IDENTIFY infectious individuals through Enblock swab ops. Amenities and facilities are FRINGE areas that the virus has the benefit in penetrating harms to human in big ways. The key is HOME, it precedes all and home is the resting and nesting place for communicable diseases!

From this epicenter (Red Zone), next is to expand a 5km diameter, this is the Amber zone, there will be scattered minor clusters of no more than 8 per cluster in developments. The next ring of fire is at 10km to 15km which is out of the critical infectious zone. However, the intersecting diameters of other ring of fire from other epicenters will intersect too. 

The idea of set, subset and intersect. If we expand this topography infectious map, we will have many rings of fire intersecting each other at various level of alert namely RED (inside 5km), AMBER (5km to 10km) and GREEN (10km to 15km). These rings are valid 30 days or less as it is an evolving virus that transit and transmit!  The TT data on physical human movements couple with MND, Statisticians, MOH are relevant resources and parties in plotting such a critical ariel view map in a continuous process to qualify and quantity areas of attention to a minimum level of 50% confidence!
