My close friend comments :-
(1) UK targets to do 100000 tests per day.
(2) What it means is that the longer we delay aggressive testing the more we have to do to catch up because the silent spread is exponential on a daily basis.
(3) In mar south Korea started aggressive testing on a scale of 10000 per day. Our DMS on the contrary said yield little result.
(4) But every asymptomatic case that slip by can turn into a thousand cases in 10days. When it quietly spread until beyond control, we are finished.
(5) Really hope sg is doing aggressive testing.

My comments :-
(1) Yes. Your suggestion of aggressive testing is the right strategy to weed out the unlink cases in Singapore - which remain a stealth reservoir in the Community that are stealthly and silently infecting unsuspecting people in the Community.

My close friend response :-
(1) It's about maximising outcomes, minimising disasters. unless we are poor and cannot afford it. Economic shut costs much much more.

My comments :-
(1) Aggressive testings is the right approach, but to test all 5.6 million residents in Singapore at one go - is a very challenging and daunting tasks.

(2) Thus we need to priorities our testing, I suggest that :-
a. Batch 1 - Dorms for foreign workers are now undergoing aggressive testing. Majority of the infected people have been identified.

b. Batch 2 - Foreign workers outside the dorms are undergoing tests now concurrently - and this form the 2nd largest group after the dorms reported every day.

c. Batch 3 --- Note :- the community local transmission and the unlink cases that are reported everyday --- are coming from this group.
But this group are all the residents in Singapore - citizens, PRs, Long Term Pass Holders. --- which form about 3 to 4 million people.
To conduct aggressive testings to this large pool of people to weed out the 30 to 20 over cases --- will be a challenge.
So I suggest we do the following :-

(i) Batch 3a - Test all essential workers that are working now - as some unlink cases come from this batch - about 300,000 workers.

Batch 3b - Test all nursing homes - where some unlink cases - have been uncovered through this batch - number unknown - but not a big number, I guess.

Batch 3c - Test all pre-school students and staff who will be attending pre-school once circuit breaker ends - as some cases also come from this batch - number unknown --- but i guess quite a big number.

Batch 3d - Test all workers that will be going to work for non-essential services when circuit breakers end - as few cases are reported from this group - a big number - potentially about 1 to 2 million workers?

Batch 3e - Test all school going children eg. primary, secondary, JCs, IHLs - that need to go to schools and pose a risk of cross infection - before circuit breaker ends - potentially about 1 to 2 million students?

(ii) Note :- by testing all the Batch 3a to 3e ---- if there is any discovered cases --- their family members or those living with them or come into with them will be tested --- and these will have weed out the unlink cases.

(iii) Can we take the risk of not testing all those staying at home --- yes i think we can --- because if the above batches are tested - with no infection - very likely that those stay at home are not infected.

(iv) If anyone of the tested batchs have people infected, then likely that those stay with them will be infected - and this stay at home can be tested.

(v) Believe in this way --- we have a systematic aggressive way of testings to weed out all the unlink cases in the Community.

We put a caveat to the systematic testing.
If the community spreading and local transmission consistently drop into single digits to zero, then we suggest not begin with the systematic testing - as the Stay at Home and social distancing have shown 100% effective and can dispense with aggressive testing.

However, suggest that we should continue testing for those who are in dorm as well as those foreign workers who are not in dorm as they are still many reported cases.

Singapore has changed the dynamics of its care of Covid-19 patients by treating nine in 10 of them in community facilities instead of acute hospitals.
The move not only differentiates those with mild illnesses from others with life-threatening conditions, but also saves the hospitals from being deluged.
Community facilities now house 9,878 patients - out of the 11,107 current patients. This means just 1,229 patients are still warded in acute hospitals, including 24 in critical condition.

Analysis of our current covid situation :-
(1) Today though reported 897 cases - the infection numbers are coming down.
(2) 13 cases are local transmission - the lowest recorded compared to few days ago.
(3) So indeed the circuit breaker measures are reaping positive outcome - as we can see that local transmission are coming down based on the past few days numbers.
(4) The dorm figures are also coming down from thousands to a couple of hundred with aggressive testings.
(5) Notice that 8874 have been moved from hospitals to isolation houses and only 1368 need to be in hospital.
(6) This indicates that 8874 are mild cases - that do not need active medicinal intervention and can recover by themselves.
(7) Only 1368 need medical attention while 25 need ventilators support.
(8) So all these are encouraging signs - that we are slowly turning our tide and fight against the covid virus.

Pleased to see that community cases are dropping. The next step is enlarged the test criteria for the community to institute testing for
(1) those under quarantined/SHN before lifting their movement restrictions. When the community cases start to clear up, there is no reason to release those close contacts without testing them.
(2) occupationally higher-risk groups should be tested once they showed symptoms e.g. Public facing personnel, healthcare support workers etc

The foreign workers new cases appear to be sliding but the absolute number is still high means infection rate potentially still very fast. There will be some way to go before we see lower numbers. Once the symptomatic workers are segregated, we can practise random testing supposedly healthy group to check on the infection rate. E.g.. Sample x number of workers everyday picking from different floors and different blocks to monitor the infection level.

[7:03 pm, 25/04/2020] : Reach, it is quite unfortunate Minister of Manpower failed to understand or refuse to appreciate the frustrations of the employers. Two key issues. First the constant changing of the goalposts by the Task Force (example is the use of masks) frustrated many. The decisions should have been tight and severe and done once. Second, the little time given to the contractors who had little leeway to find food in a hurry. It was unfair to the contractors. Just because the Task Force realized to late, they just jammed the emergency brake. Frankly, worse of all, the Ministers in the Task Force did not apologise for their shortcomings and failure to foresee some of the occurrences. More reactions rather than pro- actions. These are my frank views of the situation.

It is easy for those critics especially oppositions to sit behind a pc to start shooting because they don't need to fight the battle but just need to find fault.

And by doing so I got a better picture of my situation. One dentist was out to ‘scam’, another dentist claimed a procedure could not be done but actually it could (as of now it’s already done). So that’s why we cannot totally trust any one expert but have to do some critical thinking and maybe consult more experts, in maybe more than 1 field, to decide on the most optimal course of action.

(1) a. Some people may think - this virus won't get me or my family - not so 衰。
b. Some people may say - I get it - my business - it is my health.
c. Some people say - I lazy to change my routine - as long as others take precaution - I go along with my normal routine I won't get it.

(2) Tha actual fact is :-
a. Virus got no eyes and don't discriminate or choose who to infect.
The moment the virus come across someone who don't protect themselves or who don't stay at home and isolate themselves from others who are infected - the virus will strike.

b. Some may think, my health is my business. My body is strong, I can withstand the virus.
But the problem is, when you get infected, you also infect your loved ones - your wife, your husband, your children, your parents, your friends, your colleagues, the community and the world.
Your responsibility is not "ONLY YOURSELF" --- but the everyone as a WHOLE.
One-For-All, All-For-One.
You have become a sinner - if you wilfully breach the circuit breaker - got yourself infected and infect others.

c.Everyone is changing their routine to make difficult adjustment - in the hope of defeating the virus or at least keep the virus at bay --- until the vaccines are found or standard treatment are developed to protect everyone.
If people cannot make sacrifice by making adjustment - to jointly defeat the virus --- then we fail ourselves and fail the country as a whole - as billions of reserves are used to tackle the virus, make everyone safe ---- while many sacrifices their business, job, salary and personal freedom to make the circuit breaker works.
A small sacrifice on our end - is it too much to ask?

3. When a person are infected with the covid-19 virus - what a long term effect a person will have to suffer :-
a. lose the taste - as the taste cell is damaged.
b. lose the cell that protected the lungs - as the lung lining cell is damaged.
c. risk their own lives - as cells that help the lungs breathe are damaged.
d. daamage the red blood cells - by causing blood clot - as the virus damage the red blood cells.
e. confused the white blood cells - and cause the white blood cells to misbehave

and many other damages to the body organ and system.

4. So why people don't make some sacrifice to protect themselves and their family and the community - by observing the circuit breakers --- instead of wilfully break the circuit breaker --- and cause the above trouble that can be avoided?

Notice that :-
Hospital 1368.
In isolation 8874.
Most have move out from hospital to isolation facilities.

I think those who move to the isolation facilities are mild cases and need little treatment or medication that can recover by themselves.
The 1368 probably need medication like cough medicine, running nose etc.
Only 25 need special cases and ventilators.

I suspect gov needs to acquire/set up more isolation facilities

Oh very big 4000 plus close to 5000.
Now Singapore is building a 15000 beds in tanjong pagar terminal to use as isolation for those mild cases that doesn't need medical care.
Ship berth terminal at tanjong pagar have move to Tuas megaport liao.

We should correct those who spread incorrect information. But be wise in putting efforts in defending opinions.
The war is not over or worse far from over. Focus on the desired outcomes : -
(1) are the infected able to receive appropriate treatment? Answer YES
(2) are other patients able to receive appropriate treatments. They are not denied of their treatments. Answer YES
(3) is the covid death rate amongst the lowest in the world? Answer YES
(4) Any social unrest? Answer NO
(5) Any political crisis? Answer NO
(6) is our recession one of the shortest and least painful around the world? Answer YES
(7) when we rebound, can we maintain and continue to clinch the top spots in world rankings? Answer YES

Then the rest is legendary - get media to write good stories for singapore and it will remain in wiki pedia for very long
For now, we are skating on thin ice, many dangers and we may tip over for the worst.
We can only highlight, provide constructive ideas and let gov and partners do their jobs.
As citizens we must also do our part.

As of 12pm, MOH has preliminarily confirmed an additional 897 cases of COVID-19 infection in Singapore.
Majority are Work Permit holders residing in foreign worker dormitories.
Of the 897 cases, 13 are Singaporeans/PRs.

Analysis of our current covid situation :-
(1) Today though reported 897 cases - the infection numbers are coming down.
(2) 13 cases are local transmission - the lowest recorded compared to few days ago.
(3) So indeed the circuit breaker measures are reaping positive outcome - as we can see that local transmission are coming down based on the past few days numbers.
(4) The dorm figures are also coming down from thousands to a couple of hundred with aggressive testings.
(5) Notice that 8874 have been moved from hospitals to isolation houses and only 1368 need to be in hospital.
(6) This indicates that 8874 are mild cases - that do not need active medicinal intervention and can recover by themselves.
(7) Only 1368 need medical attention while 25 need ventilators support.
(8) So all these are encouraging signs - that we are slowly turning our tide and fight against the covid virus.

1 There is a much more efficient and better way to do this. China has showed the way and we shared in this forum on 3 Apr. (Understand that it will take some time to develop the apps if consider implementing this solution).

2 Gov should maintain a central database of daily declarations and basic information. Each time we need to go out, we will have to declare. Questions can be (i) recent travel history (ii) recent contact history of people infect with covid 19 or quarantined (iii) experiencing covid symptoms (iv) does your job require you to come into contact with general Public etc.

3 Each business premise should be assigned a unique QRCode. This can be derived from UENumber of that business entity.

4 every person has to install an app on our mobile phone. Make a declaration for that day in the app.

5 each time I enter a premise, I have to scan the QRCode and this will be captured as a trace that "I was there". Date and time stamped. When I leave, I scan the qrcode again date time stamped that I left. This log will be captured centrally as well as cached within the app.

6 during contact tracing, enforcement officers can send out alert that an infected person was confirmed and should I display symptoms I should go for tests.

7 example of how this is going to work during the CB. I need to buy grocery at supermarket. I make a declaration if i have not for the day.I arrive at NEX shopping, scan NEX shopping mall QRCode. Head towards NTUC fairprice, scan QRCode for ntuc fairprice (NEx). When I leave NTUc fairprice, scan to "checkout" from fair price. When I leave NEX, scan to to "checkout" from NEX.

8 this pandemic may last for much longer (could be a year). As our situation improve, more businesses premises will be included. This can include office buildings, and individual tenant offices.

9 it is not fool proof, but a relatively easy implementation.

I think there is still plenty of people still not aware that children below 2 are not require to wear mask.

My experience and challenge faced:

Yesterday, as usual, I performed my normal daily routine, sending kids to school then head over to the market to buy my breakfast.  After ordering my food, I stood far away to wait for my food to be ready.  And there this aunty who is working there, demanded and insisted that my baby girl needs to wear a mask.  I told her that our government had actually announce that children under 2 not require to wear mask, she then continue insisting that there is shops nearby selling mask for infants, if not, to visit the Community Centre to take one which government has distributed.

How should I handle all this kind of issue? This has been bothering me and I really didn't want to visit the market anymore as is it very tedious and tiring to keep explaining over and over again and still have to handle those weird eyes/facial expression from the people giving me the benefit of doubts why am I still bringing my baby out.

2. Just buy a wholemeal bread or white bread.
3. Buy chocolate spread (no need nutella - very expensive, just normal chocospread that taste like nutella chocolate cost $3.95) or buy fruit jam or kaya --- where children and adult also like.
4. Then go along with 3-in-1 coffee for adult, or milo or milk for kids.
5. A wonderful breakfast is done for everyone - less than 30 minutes --- nothing difficult - and everyone is safe - still can teach the kids how to prepare a simple breakfast for themselves - in a homely setting --- and turn on the TV or music --- to let the family enjoy a warm family.
6. Still need to go out - and endanger the young kids?
7. And everyone just need to endure this for another 6 weeks --- where hopefully the local transmission drop to zero --- a sacrifice that everyone is doing.

I will still do as per normal. If NTUC too many people, I will not go in. I will only go NTUC when the crowd is low. Ths most I buy from those small mart. Going to explore more on online learning and learn extra skills during this period.

(1) One of our Doctor spokesman in CNA say that testing all foreign workers does not mean that all are clear of viruses - even if the results all turn negatives.
(2) He say when a person tested negative for 4 days, the next 4 days, virus load could then surface indicating that he could be infected and able to start infecting others.
(3) This virus is a very sneaky enemy.
(4) The only way to ensure virus infections end is through social distancing where virus cannot jump hosts and the virus die a natural death.

My close friend's comments :- Given the living conditions at the dorm, how is social distancing possible?

My comments : Yes, that is the scary part.
The doctor say the worst scenario is where 40,000 foreign workers will be infected.
But Singapore government is trying to break the workers into smaller groups and put them into many different places like HDB, floating barges, army barracks etc.
By doing so, the number of infected workers within a group will be greatly reduced and when the virus has no host to jump - it will die a natural death - and the chain of transmission will stop.

(4) Someone mention Dr Chee or Tan CB will ask ---- has no meaning - as to put the blame on the Government - when clearly the dorm operators did not maintain the hygiene standard or the foreign workers did not maintain their own hygiene.

(5) It is akin to a household keep all the rubbish in their houses --- and infested with rats and cockroaches --- and then put blame on the government for not maintain hygiene in the houses.

(6) It is easy for this arm chair oppositions to shoot their mouth like a competent politicians - and when ask to run - is a different matter all together.

(7) And after the pandemic, will the workers be rehoused into their dorms?

(8) Answer:- why not?

(9) 20 persons to a big room - is not overcrowded.

(10) In army barracks - about 15 to 20 persons to a room is a norm.

(11) As long as the vaccines are found and vaccinated, plus hygiene is maintained in a room - 15 to 20 persons to a room is not too much to ask - as Singaproe is a small place.

(12) The only thing is that - army barracks have "stand by bed, stand by room" --- where army commander check the cleanliness of the room and hygiene.

(3) Let us monitor for another few days and see how it goes.

(5) Taiwan though experience 3 consecutive days of 0 cases - suddenly come back with a warship of sailors with double digit infection - and these sailors roam around many Taiwan counties --- and now the whole island is on alert for likely more infected cases.

Today marks 14 days since the circuit breaker measures came into effect.
As Singapore enters the second half of the month-long circuit breaker period, most of the new cases are linked to foreign worker dormitories, while local transmission in the community has continued to fall.
Minister Lawrence Wong said said some have difficulties staying at home and some are being cared for by volunteers. He encouraged everyone to stay on course as the Circuit Breaker measures are showing results.

3 wks ago, SG can do about 2.9k tests a day. It should be more now.
But the concern here is about lab space. Cos the tests need to be done in the lab. My sense is that SG has enough tests. SG has companies manufacturing their own tests as well. But having all these tests and reagents is one thing. Having enough labs to test the results is another.

The swab test is the one Govt is relying on for initial testing because it tests directly for the presence of the virus. Which makes it more accurate. And in fact all govts in the world uses the swab test for initial testing. Swab testing needs the lab.

Antibody testing doesnt need lab. But this is less reliable because it tests for antibody which is produced in response to the virus. The problem is it takes a while for antibodies to show - some papers says its a week or so. Sometimes antibody might not show where virus is in small numbers. So govt usually use this antibody testing afterwards to determine who many people is infected - ex post facto.

So I wld say that for the purposes of our case mumbers each day - these are all from the lab

3. However, wonder did Netlink sent their workers for covid test before deploying them to home even though they practised safe distancing in deployment? This is my real concern.

(2) Baring that double digit of these "foreign workers not in dorms" - are found to be infected every day, should there be a more systematic ways of testing these group of workers to identify those infected before the circuit breaker deadline arrive.

(3) Suggest to do the followings if we have spare capacity to systematically test this group of foreign workers :-
a. Mandate that Contractors will need to get "medical cert-free of covid-19" for all their foreign workers under them - before these workers all allowed to work in the construction sites or/and when circuit breaker ends.
These will put the onus of tests on these contractors if they want to start work when the circuit breaker ends.

b. Setup mobile testing team (eg. in vehicles or in CC near where these construction workers stay - eg. in HDB, construction sites, private estates etc) - where all workers staying around the vicinty must be escorted by their respective Contractors or Companies to be tested --- each and everyone -- to ensure all their workers are fit for work or if infected - to be send to hospital for treatment.

c. These mobile testing teams - can initially be deployed in North, South, East and West - to ensure it covers all the foreign workers not staying in dorms.

The moment when all the tests are done on dorm ---- the number will decline and eventually go down to zero.

During the period where the symptoms appear to confirmed it might have spread to several others and chain of transmission proliferates exponentially. When community spread has become pervasive, can the authorities consider widening the test criteria e.g. Persons who showed symptoms and worked in public facing environment should be tested.

For these apps to work, we will need everyone’s cooperation to install and use them, like what the South Koreans have done. There will be some privacy concerns, but we will have to weigh these against the benefits of being able to exit from the circuit breaker and stay open safely.

(2) We need to move ahead and tackle the virus situation heads on.

(3) We need to move from a "evidence based" approach to a "risk based management" approach - to tackle this virus pandemic - as it is a very "stealth" and "highly contagious" enemy - and move with speed and stealth - that will catch human offguard.

(4) As we admire Taiwan handling of their virus situation - we also noted that one of their warship got infected - and double digit infection suddenly exploded.

(5) So we do have up and down - in tackling a sly, stealth and fast spreading enemy --- and when we are down - we don't start blaming - but to tackle it heads on.

(6) Remember this is a marathon, once we lower our guard, the virus will overtake us.

(7) Now we just need to stay focus our energy to catch up and overtake and overcome this virus and keep our number down.

Lately community figures decreased, i am wondering if it is because most of the testing are done for the fw dorms and less testing done for community (afterall there is a limit to testing capacity), which is why the community numbers decrease?

[3:31 pm, 20/04/2020] : i m of the view that as long as our local cases are low and continue to go down (single digit or 0 (better)), we should gradually lift the circuit breaker by the deadline.  We manage and control the dorm and foreign worker cases by isolating, tracing and treating to prevent it from spreading in the community.  A lot of things cannot wait, and an extension will compound that.

[8:37 pm, 20/04/2020] : Reach, this posted list is quite serious and shows that the infection within the FW community is our biggest challenge. My suggestion is to, once and for all, isolate all FW quarters. All means all. And test 100% and further  isolate the infected ones. Otherwise, our local community will not be safe as you can't prevent interaction between FW and locals. I also noticed the infection rate among our front end health workers is getting worse. I hope we can find out the flaws and fix them quickly.

I think the thing most people don’t understand is that the lock down is in place to slow the progression of the disease, NOT to eradicate it by end of May.

I think people are missing the point because they naively think the govt will somehow get rid of it so life goes back to normal. These are people who have ignored the global reality since January. This is the dangerous delusional thinking that some people are applying in their assessment of the situation when advocating we should stop the lockdown because they are literally only waking up to this thing now, or when we were initially shut down. So let’s lift the curtain a bit.

1. Flattening the curve means slowing down the disease so that hospitals can cope with the sick. It does not mean eradicating the disease all together.

2. No government will be able to eradicate the disease this year. It’s here until a cure or a vaccine is found

3. A vaccine is at least a year away. That’s April 2021. The earliest vaccine trials just started late last month. It takes 18 to 24 months to make, test and assess the efficacy of a vaccine. So 1 year is literally a Hail Mary.

4. There is NO going back to normal. Your normal will not exist for at least a year. Economies globally will keep bleeding and we are likely to have rolling lockdowns until a vaccine is found.

5. Even when the ban is lifted, tourism and hospitality industry, and many others will not see an increase in business for at least a year. Until a vaccine is found people will continue to self isolate even when a lock down is lifted.

6. The only reprieve we are going to get is if we have thousands of people who have been infected and recovered and minimum deaths so that we have some level of herd immunity. That is also still a theory that is yet to be proven because some countries think they may be seeing people get reinfected. Even with that in place normal is not feasible because fear still reigns and people will remain isolated until safety is guaranteed- cure or vaccine. Refer to point 3.

7. The above is still 6 months away. We are projected to reach our peak in September. That means, bleeding economy, rolling lockdowns, death and chaos until then.

8. The decisions made in the lockdown like no alcohol and no dog walking or running and no e-commerce are not made with the privileged in mind. They are made with the masses in mind. What you allow in the suburbs you must allow in the townships and those two things look very different in execution.

Now our government is doing the best it can and they have reacted faster than most developed countries. Our progress is still better than most and right now we’re riding the dumb hope that somehow BCG gives us some fighting chance (still unproven).

So please please we all need to do whatever we can to help our neighbors, help each other, support our government and stop having delusional ideas about any level of normalcy at the end of this month. Adjust your projections and hunker down.

This is not a joke and it’s not even anywhere near any semblance of real yet.

Normal is dead. Let that sink in. But HOPE  is very much alive!
